If this is the case, then there is a REAL problem!  This is an issue 
that should focus on inventory control, receipt and inspection, 
process control and the ability to recognize that appropriate 
controls are in place, and more importantly, when there are no 
controls, how is the data evaluated using statistical accuracy, and 
not simply throwing in a fixed subtraction into the calculation. The 
fixed value should really be a variable value, that incorporates the 
time-frame and other important influence quantities. Tim's suggestion 
is a symptom, but the real question is, what are the root causes for 
this to even happen!
