Look at IAEA Safety Series No. &, Lessons Learned from Accidents in Industrial Radiography., 1996.  You can read almost identical scenarios there with carrying the hoses around the neck.  

Failure to wear an alarming dosimeter, failure to use survey meter, etc.  The causes never change.
