[ RadSafe ] Sealed Source Recovery at the University of Washington

Two questions:

1) Since the University is a state licensee, why wasn't the State DOH
supervising the operation?

2) Why wasn't the manufacturer of the source and irradiator, JL Shepherd
called in to retrieve the source?  After all, they know most about the
design and fabrication.  They would have been unlikely to saw into the
source capsule.
