Washington issued reciprocity without asking what the company was going to
do??!!   This is unbelievable.  How can they grant reciprocity without
knowing where they were working in the state and what they were doing?
Before granting reciprocity the state must determine that the company's
license authorizes the activities to be performed.  Furthermore the
University was obligated to informed their regulator that they were
disposing of the source, when and how.

I did not read the entire report yet.  I hope the above factors were listed
among the root causes.
