Try NUREG/BR-0024, "Working Safely in Gamma Radiography", Dr Stephen McGuire. You can get NRC NUREGS by calling (202)512-1800, or at the National Technical Information Service, (800)553-6847. It is an outstanding text and includes several color photos illustrating what can happen if a radiography source is mishandled.

I am currently searching for some fairly graphic photos of high level
exposure from sources, (radiography sources, etc.) for some source
accountability training I'm giving. If anyone can point me to an
appropriate internet site or e-mail an attachment I would be extremely
grateful.
