RE: Mallinckrodt didn't take radiation overexposures seriously,inspectors say

RadSafers,
One question I have when reading posts about this issue is: how often does
the NRC inspect this facility? How come the "trend" was not discovered
earlier? These are not rhetorical questions, I am really curious. Maybe I
just missed some details that have been given somewhere.
