internet source for problems at irradiators

Greetings Radsafers:

I have an RSO at an irradiator licensee that is interested in finding a
possible source via the internet that will inform him of recent accident,
mistakes, or problems at irradiators. The irradiators I am talking about are
the MCi ones that are used for sterilization of medical supplies. However, I
believe that a broader incorporation might be acceptable to him.
