I found the following two articles.
Sounds like a lost source holder from industrial radiography.

Worker took home radioactive cable by mistake
A Chinese factory worker has won compensation after losing both legs and
part of an arm after taking home a piece of radioactive metal by mistake.
Song Xuewen pocketed a fragment of metal cable as he walked through his
factory, not realising it was highly radioactive piece of equipment.
He fell ill within two hours and was given permission to go home. It was
only later the same day that company inspectors realised what had happened
and a team was sent out to retrieve the radioactive material.
Since the incident at the Dandong Radiation Factory in 1996, the worker has
undergone seven operations and has also become impotent.
Awarding 500,000 Yuan (about £42,000), the court found the plant's parent
company responsible for not properly storing and guarding the cable,
reports the South China Morning Post.
Both parties have appealed against the outcome of the hearing. Although it
is one of the highest awards made to an industrial accident victim in
China, the victim had hoped for more.

Worker Gets Compensation for Radiation Sickness
SHANGHAI, Nov 20, 2000 -- (Agence France Presse)
A Chinese worker who lost both his legs and a hand as a result of nuclear
radiation received CNY 487,837 (USD 58,917) as compensation, the Shanghai
Evening Post reported.
Song Xuewen, a laborer at the Jinlin Chemical (Group) Construction Company,
picked up a small metal chain from the factory floor on the morning of
November 5, 1996 and put it in his pocket.
The chain emitted such high doses of gamma rays that Song lost both legs
and his left forearm and was left sterile from radiation sickness after a
few hours exposure to the contaminated metal.
After seven operations, Song sued the company for CNY 5.8 million, the
paper said Sunday, giving no details on how the highly radioactive metal
had found its way into the construction company.
Song was awarded CNY 487,837 in compensation by the Jilin Provincial Higher
People's Court, in additional to his medical costs, the paper reported.
The bill for Song's medical expenses of CNY 450,141 was split between the
Jilin Chemical Construction Company and the Dandong Rays Instrument Group
which paid 40 percent and 60 percent of his bills respectively, the report
added.
