We seem to be missing the point. The issue is not whether there is an 

incident in nuclear medicine, reactor or industrial radiography. The 

issue is "human errors" and taking corrective actions. There is no 

excuse (or reason) for human errors. To correct these type of errors 

one has to rely on appropriate procedures, appropriate training and 

appropriate oversight. 



It seems so simple, why do we seem to be losing sight of the bottom 

line? Errors should not be tolerated, simply dismissing them as 

having no potential for a significant dose. Perhaps that is true in 

the particular case, but, there is always a potential for a larger 

problem later, if the true root causes aren't determined, and, 

eliminated.
