NRC INFORMATION NOTICE 95-43: FAILURE OF THE BOLT-
LOCKING DEVICE ON THE REACTOR COOLANT PUMP TURNING VANE.
NRC issued this information notice to alert addressees to a
recent event involving problems that have resulted in the
loss of integrity of bolt-locking devices in the turning
vane diffuser attachment of Westinghouse reactor coolant

Licensee Event Reports. The FULL TEXT of ALL LERs
(from #1 in 1985 to present) are now available for searching
on our BBS. Telnet to 204.188.70.1 or dial (510) 547-0598
(8N1) to log on our BBS. After logging on the BBS, select
"1" from the Top Menu and "2" from the next menu. Then
specify key words you wish to search for in the space
provided. The files are too large to download, so that
feature has been disabled in this service. (You can, of
course, still "download" by using the screen capture feature
of your communications software.) The database for 1993 is
slightly corrupted, so it will not read well on yout screen
(we'll fix this as soon as we can).
GINNA. The Ginna plant experienced minor leakage from
its spent fuel pool (SFP). The leakage is evidenced by
boric acid and activity present in water seeping into the
residual heat removal pump room (which is located
immediately adjacent to and below the SFP). The leakage
rate is very low and does not present an operational
problem. Attempts to quantify the amount of leakage are
complicated by (1) evaporative losses from the SFP, (2) the
extremely low leakage rate (seepage), (3) ground water,
which is contributing to seepage into the RHR pump room,
and (4) boric acid, which interferes with chemical analysis.

LIMERICK. On September 11, 1995, while operating at
100% power, PECO Energy manually scrammed Limerick 1 due to
a stuck open main steam relief valve.
The failed valve is a two-stage Target Rock SRV, and
was one of five SRVs PECO had identified as leakers. During
the event, the "A" RHR pump showed abnormal characteristics
when running in the suppression mode cooling mode of
operation. The pump was secured, checked, and restarted
without unusual noise at a flow rate of 8500 gpm. PECO used
both the "A" and "B" RHR loops for the suppression pool
cooling. PECO removed the failed SRV and sent it to Wyle
Laboratories for evaluation. PECO plans to replace all five
SRVs identified as leakers.
PECO made an initial evaluation of the "A" RHR pump
suction strainer and determined that one side was completely
clogged, while the other side was partially clogged. GE
will analyze the material removed from the strainer by the
initial diver. The diver reported that visibility was not
good to evaluate the entire during his dive. PECO is
assembling a team of divers to complete the pool cleaning,
and the required inspection, and cleaning of all strainers
in the suppression pool. (9/29)
MAINE YANKEE. NRC held a public meeting in Wiscasset,
Maine on September 14, 1995. The purpose of the public
meeting was to receive an update from MY on its sleeving
campaign and sleeving issues, and take questions from the

VENTILATION SYSTEM CHARCOAL RESULTING FROM CHEMICAL CLEANING
OF STEAM GENERATORS" NRC is issuing this information notice
to alert addressees to the potential degradation of
ventilation system charcoal as a result of the charcoal's
exposure to chemicals during the chemical cleaning of steam
generators.
