To be a listed consulting physicist qualified to perform the annual State
required mammography inspection for California facilities, contact: Larry
Carter (916) 445-3088 (Lcarter@hw1.cahwnet.gov) for an application. Currently,
you must fill out the application and append two mammography inspections
reports from inspections you have performed. You may also request the
application in writing from: California Department of Health Services,
Radiologic Health Branch, 601 N. Seventh Street M/S 178, P. O. Box 942732,
Sacramento, CA 94234-7320, Attn: Larry Carter.
