fm NRC's Daily Report issued 11/13/95



     Interesting posts from NRC. One has to do with a damaged density 
     gauge. The second, asserts radiation overexposure due to

| A DENSITY GAUGE WAS RUN OVER BY A BACK HOE.  THE RADIATION SAFETY OFFICER    |
| WAS CALLED TO THE SCENE AND SURVEYED THE AREA.  NO CONTAMINATION WAS FOUND   |
| IN THE AREA.  THE SOURCE ROD WAS IN THE SHIELDED POSITION AT THE TIME OF     |
| THE ACCIDENT.  THE SOURCE ROD WAS BENT, BUT DOES NOT APPEAR TO BE LEAKING.   |

| RADIATION LEVELS WERE 0.8 TO 1.0 mR/HR AT 1 FOOT FROM THE GAUGE.  THE GAUGE  |
| HAS BEEN TRANSPORTED BACK TO THE COMPANY'S STORAGE LOCATION IN SOUTH BEND.   |
| THE GAUGE CONTAINS  Cs-137 AND AmBe SOURCES.                                 |

| OFFSITE NOTIFICATION - A MAN WENT TO A LOCAL HOSPITAL SAYING HE WAS          |

| THE LICENSEE NOTIFIED THE COLORADO DEPARTMENT OF HEALTH OF THIS EVENT. A     |
| MAN REPORTED TO ST. ANTHONY'S HOSPITAL IN DENVER STATING THAT HE HAD         |
| RADIATION BURNS ON HIS ARMS.  A SURVEY CONDUCTED BY HOSPITAL STAFF DID NOT   |
| REVEAL ANY CONTAMINATION.  A LOCAL HAZMAT TEAM (WEST MINISTER FIRE           |
| DEPARTMENT) HAS BEEN CALLED.  THE MAN DOES HAVE A RED SPOT ON EACH FOREARM.  |

| THE MAN BELIEVES THE JACKET WAS CONTAMINATED WHEN HE WAS ONSITE ON TUESDAY.  |
| THE LICENSEE STATED THAT THE MAN HAD NOT BEEN IN ANY CONTAMINATED AREAS.     |
| THE MAN BELIEVES HIS JACKET WAS CONTAMINATED WHEN HE TOOK IT OFF TO GET HIS  |
| WHOLE BODY COUNT.  HE STATED THAT HE PLACED IT ON TOP OF SOME VIALS.  THE    |
| LICENSEE DOES NOT KNOW OF ANY VIALS THAT WOULD HAVE BEEN PRESENT NEAR THE    |
| COUNTER, BUT THEY ARE INVESTIGATING.                                         |

| THE WHOLE BODY COUNT WAS PERFORMED IN A SERVICE BUILDING.  THERE ARE SOME    |
| LABS IN THE BUILDING FOR PERFORMING COUNTS ON POTENTIALLY RADIOACTIVE        |
| MATERIAL.  THERE ARE NO KNOWN CONTAMINATED AREAS.  THE MAN EXITED THE SITE   |
| WITHOUT SETTING OFF THE PORTAL MONITOR.                                      |
