As I've heard it...
one of the turbine generators was disconnected as shutdown began
- emergency core cooling system was disconnected
- one of the control rod systems were turned off
- the other control rods were mispositioned
- control rods were driven beyond safety limits
- all 8 pumps were running risk of pump breakdown
- the automatic SCRAM systems were disabled
- computer notification that immediate shutdown was ignored
- shift manager ordered SCRAM but slow movement and a
empty section of the control rods did not prevented it
(overheating producing more steam, and the positive void coefficient)
What exactly were they trying to do that they
had to shut down every safety system? Safety tests, my ass.
