Criteria for Extremity Dose Monitoring

I would be interested in hearing how people are addressing the
following issues concerning extremity monitoring:

1)  What criteria do use to determine the need for extremity monitoring?

2)  If criteria is based on dose rates, i.e. ratio of WB to extremity, have
you correlated your instrument readings with your extremity dosimetry (I
would be especially interested in any correlations done with RO-2 's
and Vinten tape dosimeters)?

3)  At what frequency do you process extremity dosimetry, i.e. after
each use, when WB badge is read, monthly, quarterly, etc.
