offers LSO training.  I went to one of their courses about two years ago.  
E-mail me privately if you want more info about my impressions of the course.

I've put a laser safety training tutorial for UIUC users on the web at:

if anyone is interested.  I would also appreciate constructive criticism in 
regards to the tutorial (though I can't guarantee that I'll take it <grin>).  I 
plan on incorporating some graphics and photos into it at a later date.
