lessons learned

The purpose of this posting is to solicit feedback regarding the value of
trending, for the purposes of improving operations, of radiological
incidents that occur at your own facility or within the HP community as a
whole.

I have been tasked with determining whether the health physics world uses a
collective or independent (by organization) method to share lessons learned
from trending of events that are experienced either at one's own facility or
across any organizational unions. Or, to determine if the community finds
more value (if any) in learning from the merits of individual incidents.

Please return your completed questionnaire to Jeff Eichorst either by e-mail
at myself@lanl.gov, or by fax to (505) 665-6977. If you have any questions,
you can reach me at (505) 665-6980.
