If you know the source's manufacturer (probably not the same as the device),
and the source's model number, you could check the sealed source and device
registry at the NRC's website:
http://www.hsrd.ornl.gov/nrc/ssdr/ssdrindx.htm
The SS&D sheet normally will identify whether a source is "special form" and
what the special form certificate number is in the "description" section.
