Was this the  main reason for so high number of misadministrations in the
United States, for fiscal year 1996, according with Report to Congress?

involved medical brachytherapy misadministrations, and five involved
radiopharmaceutical misadministrations.

United States is a great democracy and inform openly this information, I
doubt any other countries do the same.
