Using Bob's data the formula would be:

y=1028.4e-4E-5x

where:  x is feet
   and y is in millibars

I don't have a formula, but...

Surface pressure - about 1,000 mb (millibars)

Approximations:
850 mb - 5,000 ft
700 mb - 10,000 ft
500 mb	- 18,000 ft
300 mb - 30,000 ft
