Germany: 

There's no differentiation between nuclear industry
workers and medical workers. You work with
radiation -> you'll get an dosimeter -> your dose will
be recorded.

Licensing authorities may grant exceptions if the received
dose of a worker definitely can't be more than 1 mSv per
year. On the other side there's now dosimetry for workers
with exposition to significant natural radiation, too (i.e.
flight personnel (for which doses may be "calculated only")
or waterworks personnel (severe Radon exposition).
