[ RadSafe ] Radioagraphy incident in Chile

With reference to the recent request for comment on the current safety
performance in the industrial radiography industry, this IAEA document gives
an excellent report of an incident in Chile, that clearly shows that
accidents can and do happen, and the consequences of not following clear
procedures to verify the position of a source at all times during and after
radiography measurements are graphically demonstrated.
