Absolutely concur with everything Neill states. I am not aware of any processor who absolutely requires the use of an SSN. A unique identifier would be essential to ensure database record correlation, especially if a facility wants to include records from various locations/accounts (per the requirements of the client with appropriate privacy approvals and acceptance. In my many discussions with various state regulatory entities, I have never run into any that stated a SSN had to be used. They all said that a unique identifier that the regulated facility could then link to their employees was all that was necessary.
The use of an SSN is a relic of the past. Now the NRC Form-4 and Form-5 reference to SSN needs to be revised. That would eliminate much of the confusion that exists.

I am sure that Landauer does not "require" ssn's. It is not the dosimetry
vendor that requires ssn's or allows substitutions, it is the state and fed
agencies. Feds definitely allow the use of other unique identifiers. You
should make sure your state is happy with what you are doing before you make
any changes. But Sandy has identified the key requirement: that the end user
can accurately match dose records with an individual. But as a dosimetry
vendor, I can guarantee that any vendor will supply dosimetry labeled as the
customer desires. In addition, any vendor has the ability to suppress ssn's
from dose reports.

Not only are SSNs not required to be used as ID numbers on dosimetry
reports, they definitely should not be used, in order to limit the potential
for identify theft. While Landauer still wants SSNs for their records, you
can instruct the company not to list SSNs on their reports, and have them
use some other unique ID no. that you provide them I have seen companies
use driver license numbers, but I prefer company-issued ID numbers.
Whatever you use as an alternative to SSNs, all you need to do is list "OTH"
in Item 3 of Form 4/5, as allowed by the instructions included on the back
of the forms. Equivalent forms provided by the state of Texas, and other
Agreement States also allow use of "OTH" ID numbers.

Hi team:
I know this has been addressed in the past years. but i kinda missed the
final outcome. Landauer still uses the SS# for personnel on their records.
NRC form 4/5 uses the ss# . for filing termination reports and also the
annual reports for personnel, how are you all handling the paperwork. how
are you all storing the info for security reasons?
are we required to enter the ss # in the forms? can we just keep track with
the partcipant no. as opposed to ss #.
just wanted to get a feel how other facilities handle this.
any any info or comments are appreciated.
