[ RadSafe ] Unacceptable Reason For A Dose

Unfortunately, I think that the 12 years since the Tokai-mura criticality accident have not seen major improvements to worker training in the Japanese nuclear facilities. This latest report from Fukushima would indicate that workers do not completely understand the risks that they face. 

This was specifically cited as a contributing factor in the 1999 incident. You can read more about that incident in:
