I suggest you take a look at Nuclear Chemical Engineering by Manson Benedict, et al.  My copy is second edition.  Section 4.1, pages 391-96 provide a good, basic explanation of Tritium production in various types of reactors due to neutron activation in the borated coolant and in the boron-contaminated moderator and fuel assemblies of light-water reactors, and from the Deuterium in a heavy-water moderated CANDU reactor.
