I see some non-trivial technical and political issues. Just to pick one, neutrons plus U-238 equal, in varying amounts, fission fragments and plutonium. I can see people being unenthusiastic about an engine that spews fission fragment as it goes. The fact that the engine would become progressively more radioactive the more hours of use is on it would be an issue, too.
