Bill,

If a facility would take a serious look at their operations and their 
entire program in general, and evaluate what potential risk they have 
where failures may occur, and then assess the risks (both real, 
economical and social), then appropriate actions can be taken. 
Analytical tools such as FTA, FMEA, EMEA and other well established 
techniques should really be used (where feasible), instead of just 
shooting from the hip.
