which is the LANL library link to the report LA-3611 "Review of Criticality 
Accidents" by W.R. Stratton. Although published in 1967, a revised edition 
by David R. Smith was published in 1989. That report number is DOE/NCT-04 
and the document states that the report is available from the "Nuclear 
Criticality Information System" at Livermore.
