The Cormack & Shearer "Radiation Exposure from Patients" spreadsheets are
available via anonymous ftp from ftp.flinders.edy.au,  cd
PUB/medphys/CCExMac (Mac version) or PUB/Medphys/CCExPC (PC version).

I would strongly recommend that you obtain a copy of the Cormack and Shearer
article I referenced, because it lays out all the assumptions and formulae
used in the derivation of the spreadsheets.  What we liked about the
spreadsheets was that instead of using the default values, we were able to
substitute our own values from actual experimentally measured doses (using a
tissue-equivalent source phantom irradiating tissue-equivalent adult and
infant target phantoms) that we had done locally.  We are in the process of
submitting that work for publication.
