Liz,

As far as I know, the NRC (REIRS) does not provide the individual's 
dose upon request. Each facility where monitoring was provided, is 
responsible for providing the dose information, per an employee's 
request directly, or, via an new employer request (with the 
appropriate individual's signature as well). The NPP use PADS 
(Personnel Access Data System) for all practical reasons, for 
maintaining transient worker doses. Each facility is required to 
enter estimated doses as well as the officially determined dose, 
within specified time frames. Training, medical and security 
information is also entered.

However, an individual's information is not entered without the 
written consent of the individual. They always have the right to 
refuse to have their dose, as well as other pertinent information, 
maintained private. Obtaining the signature is a key element.
