The dose is dropping in nuclear plants, but look at the contracts that handle the many of the routine high dose jobs, the reactor coolant pump and refueling guys/ladies.  This group makes it's living going from power plant to power plant doing the high dose stuff.

I recall seeing 10-20 people, at least, greater than their lifetime/age for a recent refueling outage.  The procedural requirements at the time required some extra approval to allow the individual to be issued dosimetry, either the RPM or plant manager.  Further restrictions ensued as their annual exposure crept towards 5 REM.
