In the February 2002 Operation Radiation Safety publication (starting on
page S9) is a very applicable article by Rock Aker. It documents his
experiences during the closure of Zion Station. He discusses the aspects of
employee interviews in Appendix C.

As part of decommissioning activities at the Fermi 1 nuclear power
plant, we are developing a questionnaire for former employees, to
obtain information about past activities with the potential to
impact
decommissioning.
We would appreciate suggestions for questions to include. Some
examples
are:
Are you aware of any site locations used for waste disposal?
Do you recall any spills of radioactive or hazardous materials on
site?
Please post your suggestions on RADSAFE or send them directly to me;
