Many of the significant accidents have been investigated by the IAEA.  Copies of the reports are available from the IAEA.  Some can be downloaded for free in pdf from, these include:
Gilan - Worker exposed to Ir-192 radiography source
Samut Prakarn - 10 people exposed to dismantled Co-60 teletherapy source
Sarov - Criticality accident
Tomsk - reprocessing plant accident

Other are not yet available in pdf format, but can be purchased directly from the IAEA.  These include:
