Radiation Safety Associates also has a piece of software for this. It's 

called re:Source, and is available on the website at 

www.radpro.com/resource.html .  It was developed by a health physicist 

specifically for the purpose of tracking sources and maintaining inventory 

control and other statistics regarding the sources, and it has a number of 

built-in, printable reports.
