Celia,
About 20 years ago, radiology departments did track
exam repeat rates, e.g., film too dark, too light,
motion, collimation, etc. Our error rate was ALWAYS
less than 5%. With digital imaging, it is harder to
see the errors. But technologist do make mistakes.
