The Tengs et al. (1994) report is now being reviewed for publication in
Risk Analysis.  You can get a copy of the report by calling the Harvard
Center for Health Policy and Management at (617) 432-1090.  

Dr. Tengs said that if you want to know the cause-specific deaths rates
and years of life lost for various activities (and their interventions),
you have to look in the original documents that are cited in their paper.
