Someone asked recently for e-mail addresses for ACMP, ASTRO, and JCAHO.
Here is a partial answer to your question:

The executive director of ASTRO is Nick Croce:
      croce@acr.org

The executive director of ACMP is Laura Jones:
      lauraj@acr.org

As you might guess, the headquarters of both societies are at ACR
headquarters in Reston, VA.

I cannot find (yet) an e-mail address for the JCAHO.  Their headquarters
are:

Let me take this opportunity to tell you (even though it may not be
relevant here) that I have been appointed the "Primary Contact" between
the physics organizations (AAPM, ACR-COP, and ACMP) and the JCAHO.  The
appointment was made by the Trilateral Committee in an attempt to limit
the number of physicists who contact the JCAHO, to ensure that we speak
with a single coherent voice on matters of interest to physicists, and to
provide the JCAHO with a single contact person when they have questions
or concerns.  We have identified "secondary contacts" in each society:
Steve Graham for AAPM, Marilyn Wexler for ACR-COP, and Ed Nickoloff for
ACMP.  I act as a "conduit" and am in frequent contact with these three
physicists, and with the JCAHO.  But not yet over the internet!
