1. Scintillation fluid!
Infrequently, but Not Never, people accidently put vials in the solid waste.
It is almost always a new person, often a language barrier.  

But It Happens.

You might expect to smell it first, but that will depend on how much is
there and how deep it is in the drum, etc.

Its not a bad safety factor for the long haul, which we are all in!
