I-131 does not mostly stay in the patient.  It most leaves the patient.  
Typical uptakes are in the region of 6 to 22%.  The remainder either 
decays or is excreted in urine, feces, and perspiration.

Of course, if the patient has thyroid problems, the uptake can be much lower!
