I'm curious.  Did you ask your vendor for a reply and what did they respond?
 Also, did they lump the two possibilities into one general notation on your
report?  If so, request them to modify their reporting so there is no
question whether it was damaged-meaning they can't process and evaluate the
badge at all; or it was misused-meaning the badge was processed, it was
interpretated, but they feel it should not be used.  In the latter case, some
of the detective work mentioned in other responses can give you data that may
be useful in determining the cause of misuse.
