I heartily agree with Mike Slabeck about the value of the anecdotes I've
seen posted on RADSAFE. I haven't seen one  yet that didn't have
something to  learn from.

We worry, quite reasonably, about getting all of the big things right, but as
some of these stories confirm, sometimes a series of small issues can lead
to a big problem.

Please continue adding to my education.
