I have only two comments. They are just my opinions though.
1. Better to have one authorization or permit number per principal
investigator. Put all procedures & isotopes under the single authorization
number for that research group. One exception would be splitting human and
non-human use authorizations.
2. We had a rather sophistacated algorithm for calculating survey and audit
frequency. Mainly, it included external & internal dose hazards based on
gamma constant, beta energy and activity authorized. These were weighted
and summed over all isotopes used. Then, a laboratory safety factor was
applied. The safety factor was based the lab's audit & survey performance
record. Unfortunately, we found this was very time consuming during
rewrites and ammendments. From my perspective, it seems that survey
frequency is best set by audit & survey experience. Now, we set survey
frequencies by default to quarterly. This monitored and changed by the HP
in charge of surveys as necessary. These decisions are set with the help
and recommendations of the Rad Safety techs. They are very good at
comparing performance from one lab to the next.
