NRC forms 4 and 5 are available on line.  You can access
them through the NRC home page (http://www/nrc.gov). 
Then go through "NRC Offices" "Research" "Projects" to get
to the Radiation Exposure information.  Or, you can go
directly to the page for Regulatory Guide 8.7, Revision 1, in
which the forms are presented and download the file.  The
address is http://orvc.saic.com/rg_info.htm

Also, the NRC developed software for filling out the forms and
doing the dose calculations is available through the same
address.  It is free for you to download.  It does much more
than just print the forms.  It will calculate dose from intakes
and calculate TEDE.  Pretty cool if I do say so myself.
