Bob,
10 CFR 30.36(d) contains the "timeliness rule" for decommissioning and describes when a
plan is necessary.  There is a lot of detail in the regs,  and I think your questions are
answered in that section.  30.36(g) is also important in determining when a plan must be
submitted.
