Eric,

You have basically sumarized the incident.  A 3 Ci AmBe source was 
lost downhole.  During the fishing operation, the source capsule was 
breached.  However, the source was not identified as leaking by the 
personnel on site.  A Bureau of Radiation Control inspector 
investigated in the site upon hearing of the condition of the logging 
tool and determined the site and much of the equipment was 
contaminated.  DOE later retrieved the source capsule from the 
logging company and determined approximately 1/2 the Am-241 remained 
in the capsule.
