Informed consent for X-Ray Patients

Dear Radsafers:

I am hoping to reach fellow radsafe members at medical institutions.  We are
in the process of upgrading our computerized patient admissions system.  Our
Radiology Department would like to automatically generate a pregnancy
preceptor statement for woman who are capable of childbearing.  This
statement would include a sign off to assure/document informed consent.  

I know that I have seen sample statements from some regulatory agency or
publication, but I have been unable to locate an example now that I need
one.  Any assistance would be greatly appreciated!  Please respond to me
directly.
