Just an aside, whatever personal ID you choose to use (SSN PPN), never
issue a personal dosimeter without something!

A recent NRC inspection for our MarkI irradiator, included a intense 
review of EVERY authorized individuals dosimeter report for the previous 
12 months. In one single instance we inadvertently provided the worker
copy (which does not contain the SSN), it was immediately noted and
questioned.  We realized it was the worker copy and provided the inspector
with the employer copy.  ...there were no exposures over minimum,,in case
you wondered..
