Computerized Radiation Safety Records Management

Greetings!

J. Miller, M. Bernstein & I are preparing a presentation for the HPS
Summer School that addresses the topic of computerized radiation safety
records management.  We have prepared a simple survey that we would like
to distribute to 1) radiation safety offices that have developed or
purchased computerized records management systems or 2) programmers or
vendors that provide said systems.  The survey is only two pages long
and should take 10 - 20 minutes to complete.  The information will be
compiled and used in our presentation (additionally, we will post a
summary to radsafe).  The individual responses will remain confidential,
unless the respondent allows us to use his/her identity.

will send you a copy of the survey form.  Please indicate in your e-mail
if you prefer Word 6.0 file format or ASCII.
