radiation exposures near radioactive patients

David Engelbretson asked for a list of references we had on documented
radiation exposures near radioactive patients.  In case anyone else on the
list might like to have them, here they are.  This list is not exhaustive,
nor in any particular order.  Just a bunch of references we have compiled
over a few years on this topic.  David's original question was specifically
related to doses to children; there is one reference on this list that
appears to address that (the penultimate, I think).  The rest are more general.
