Ah, but Charles, you left off the caveat in my post ... and this is 
also the necessary ingredient. All of what I said requires "Only 
positive management can ensure this".  

However, I don't disagree with any of what you wrote in your post. 
Just your conclusion on my root causes :)  Now all can say the only 
real root cause is management, and that everything else is just a 
symptom ... but after studying and  learning from the Japanese Union 
of Scientists and Engineers (JUSE) for several years, Process Control 
and Systems (with management support) are the keys to success.
