The root cause of TMI-2 was water backing up into an air line
on the condensate system.  That started the annomally, everything
else mentioned that led to core damage were contributing causes.
BTW, regarding the block valve sticking open on the pressurizer
relief tank, the same thing happened at Davis-Besse prior to TMI-2.
The folks at Davis-Besse communicated the significant event;
however, the communication network for significant events was not
as good as today.  For a pretty good accounting of the TMI-2 event,
I recommned a book called "The Warning" by Mike Wallace.  It has
a great deal of information leading up to the accident that clearly
shows the accident was avoidable. It also has comments made by
plant variuos plant employees.  Some names may be recognizable
